Lisa J. Smith Terms of Service

Please read policies for ALL appointments, contact and before scheduling.  By calling, contacting Lisa J. Smith in any capacity and scheduling an appointment with Lisa J. Smith, you agree to ALL the terms of service set forth below and receiving your session is of your own accord and free will. 

By scheduling, paying and calling Lisa J. Smith Enterprises, LLC, Lisa J. Smith, for your session with Lisa J. Smith, you automatically agree to the terms set forth by Lisa J. Smith and Lisa J. Smith Enterprises, LLC:

• All phones sessions must be prepaid.
• It’s your responsibility to call Lisa J. Smith at your scheduled time, (248) 593-5227.   Lisa does NOT call you for your appointment.
• All cancellations must be made 24 hours before your scheduled reading or YOU WILL BE CHARGED for your appointment.  No exceptions.  Call (248) 593-5227 ASAP to reschedule.  If you’re a “no show,” it’s considered a cancellation and you will be billed for the session time you scheduled.
• All sessions are nonrefundable after 10 minutes.
• If you don’t call within 10 minutes of your session, your session will be forfeited and you will NOT receive a refund.   Lisa runs on a time schedule with other people. If you’re late, so is everyone else.  All late minutes come off your session time.
• When contacting Lisa J. Smith directly, after your payment is made, you’ll be contacted back within 24 hours to schedule your session.
• Every client must fill out and return a release/confidentiality form prior to their session (can be emailed to you) with Lisa J. Smith.  This must be returned to Lisa J. Smith before your session.

In order to reschedule your missed session you MUST:

• Pay for the session that you missed in full
• Prepay through Paypal, money order or cashiers check before scheduling your next meeting. NO EXCEPTIONS
• All cashiers checks and money orders must be mailed in time for Lisa J. Smith to receive them before your re scheduled date.
• If Lisa does not receive payment by your scheduled appointment, your appointment will be canceled and you’ll be responsible for payment for that time slot you scheduled.
• All readings must be prepaid.
• You are responsible for calling Lisa J. Smith (248) 593-5227 for your scheduled time for your appointment
• ALL PSYCHIC/INTUITIVE/MEDIUMSHIP and all forms of energetic READINGS AND CONSULTATIONS ARE PROVIDED FOR ENTERTAINMENT PURPOSES ONLY and do not in anyway constitute medical, financial, legal, personal or any sort of professional advice. By engaging in a psychic reading/session or using this site, you understand that Lisa J. Smith does not provide recommendations, advice, advice or give any directions for you to follow. A psychic reader, for example, may from time to time offer a personal opinion but this does not constitute advice.   Lisa J. Smith will not diagnose illnesses or conditions (including questions pertaining to marriage, pregnancy and death). The content of psychic/intuitive readings and the use of this site are merely for entertainment purposes and should not be relied upon.

By making payment, scheduling, and calling Lisa J. Smith directly for your session, you automatically agree to the RELEASE AND CONFIDENTIALITY AGREEMENT set forth below:

I, ___________________________________, enter into this Agreement  with LISA J. SMITH ENTERPRISES, LLC, Lisa J. Smith, therefore Lisa J. Smith (hereinafter collectively referred to as “Smith”), to confirm in this Agreement the conditions of my participation in the reading and services to be performed by Smith.

I acknowledge that I have voluntarily agreed to participate and request Smith to perform this reading/service/session.

I am aware that my participation in this reading is not a substitute for psychiatric treatment, psychotherapy, therapeutic counseling or any other form of professional therapy or medical care.

I am voluntarily participating in this reading/service and I accept complete responsibility for my own psychological, mental, emotional, medical and spiritual well-being.

I acknowledge that it is my responsibility to ascertain my own need for professional counseling and to seek such professional counseling and or medical care, as needed.

For and in consideration of my voluntary participation in this reading, I, on behalf of myself and my assigns, heirs, executors, guardians and other legal representatives, release, discharge, waive and forever relinquish Smith (including Smith’s office building, partners, agents, employees, officers and directors) from any and all claims, known or unknown, arising out of or in any way connected with my participation or involvement in this reading/service/session including, but not limited to, any information provided to me or statements made during such reading/service/session.  Further, I, on behalf of myself and my assigns, heirs, executors, guardians and other legal representatives, release, discharge, waive and forever relinquish any actions or causes of action whatsoever which may later arise, and I agree that under no circumstances will I or my assigns, heirs, executors, guardians and other legal representatives, prosecute or present any claims against, sue or seek to attach the property of Smith (including Smith’s office building, partners, agents, employees, officers or directors), and that I waive all actions, claims or demands that I now or hereafter may have, for any injuries suffered by me during my participation or involvement in the reading/service, resulting from any acts or omissions of Smith, her partners, or any agent, employee, officer or director of Smith, or resulting from the acts or omissions of any other participant in the reading/service.

I, for myself and my assigns, heirs, executors, guardians and other legal representatives, hereby agree that in the event any claim for damages shall be prosecuted against Smith (or Smith’s office building, partners, agents, employees, officers or directors) as a Smith (including Smith’s office partners, agents, employees, officers or directors), from any and all claims or causes of action by whomever and wherever made or presented for damages, including the cost and expense of defending the same.

I further acknowledge and understand that any information provided during this reading/service or any other statements made during it shall be considered confidential and shall not be disclosed except as required by law.

I have carefully read this Agreement and fully understand its contents, terms and significance and understand the legal consequences of signing this Agreement.  I am aware that this Agreement contains a release of liability and a contract between Lisa J. Smith and myself and I agree and am in compliance of this Agreement of my own free will by paying for, scheduling and calling Smith for my session.
